Programs & Services Complex Care and Inpatient Rehailitation

Complex Continuing Care patients who require nursing or custodial care, and have minimal rehabilitation needs, or are awaiting placement in a long-term care facility, are clustered on two inpatient units on the Second Floor of Hotel Dieu Shaver.

Complex Continuing Care patients who have significant rehabilitation needs are clustered in a unit adjacent to the Inpatient Rehabilitation Unit, on the First Floor of Hotel Dieu Shaver. These patients benefit from a multidisciplinary rehabilitation approach to care, but require a longer period of recuperation than those patients in the Rehabilitation Unit. They receive a combination of rehabilitation services from both Inpatient Rehabilitation and Outpatient Rehabilitation. Their discharge destination is often home. Patients are reviewed frequently to determine their potential; they may be moved to the Inpatient Rehabilitation Unit if their functional capacity improves, or moved to the Second Floor Complex Continuing Care cluster is their functional capacity declines.

Inpatient Rehabilitation patients who have rehabilitation potential, tolerate a minimum of four hours of intensive rehabilitation, and are expected to be discharged to home, are admitted to the Inpatient Rehabilitation Unit on the First Floor of Hotel Dieu Shaver. These patients participate in Outpatient Rehabilitation programs.

Complex Continuing Care

Our 2 Complex Continuing Care Units are open, caring environments providing skilled care to patients with medically complex conditions such as pulmonary problems, neurological disorders, multi-system diseases, serious wound management, and palliative care.

Our program offers patient-focused care to persons who no longer require an acute care setting, and are medically stable, but whose condition requires an interdisciplinary approach to assessment and treatment for an extended period of time.

Slow paced Rehab Program

The Slow-Paced Rehab Program is designed for patients with a diagnosis of stroke, neurological and orthopedic conditions, amputations, or generalized debilitation, who can tolerate 1 - 2 hours of therapy per day. Our dedicated staff use an interdisciplinary approach, working together to provide a range of services all geared towards assisting the patients to reach their goals and return to the community setting. By providing assistance and education, the team works closely with the patient to deal with their condition and maximize their independence.

Active Rehab

The Active Rehab Program is designed for patients who require an interdisciplinary approach to their care, participate in a minimum of two therapies such as Occupational therapy, Physiotherapy and Speech therapy, and require 24 hour nursing care.

Our educated and caring staff provide an atmosphere that optimizes the patient's comfort and independence.

Our In-Patient Program is designed for patients with a diagnosis of stroke, amputation, neurological or orthopedic conditions, providing both individual and group sessions, based on the needs and goals of each patient in order to facilitate functional well being. Patients must be able to tolerate 2-3 hours of intense therapy per day.

The patient is an integral member of the interdisciplinary team and full participation is essential to the success of the care provided.
