Position Objective:

We are seeking an experienced Social Worker to plan and provide intervention and support to patients, families and health care providers in a multi-disciplinary team. The incumbent will use their skills in psychosocial assessments, care plan development, implementation and evaluation of treatment plans for Rehabilitation, Palliative Care, and Complex Continuing Care, patients and their families.


The successful candidate will:

  • Provide services as a member of the interdisciplinary team by providing psycho-social expertise to the team by assisting patients/clients and families with their psychosocial adjustment to illness, disability and institutionalization.
  • Perform direct patient care which includes the assessment (e.g. depression, suicidal risk), identification of problems, goal setting, treatment planning and implementation and outcome evaluation for each patient/client and her/his family.
  • Have demonstrated knowledge of the Social Worker’s scope of practice including individual, and family counseling to assist patients/clients and families who are experiencing emotional difficulties, behavioral changes, family conflicts, grieving losses, adjustment to changes in life style, difficulty in accessing community resources, financial problems and discharge planning.
  • Establish and maintain a purposeful and therapeutic relationship with the patient/family to attain optimal benefit from hospital resources allocated to their care.
  • Maintain accurate documentation of patient/client assessment findings, goals, treatment progress and outcomes.
  • Contribute to the comprehensive management of patient care by participating as an integral member of the interdisciplinary team, promoting psycho-social awareness and the role of social work. 
  • Educate patients and families to strategies related to coping with lifestyle changes.
  • Be responsible for contributing to a transparent culture of patient safety by adhering to and abiding by patient safety policies and procedures.
  • Demonstrate knowledge of and commitment to safe, high quality, ethical care to all patients by following Hospital policy and procedures.
  • Possess a strong knowledge of relevant legislation (i.e. Health Care Consent Act, Substitute Decision Act, Mental Health Act, Public Guardian and Trustee, Child Protection Act).
  • Be self-directed and able to coordinate and prioritize multiple competing workload demands with minimal supervision.
  • Integrate education, critical thinking, and relevant practical experience in problem solving.
  • Possess human relations, empathy and interpersonal skills sufficient for successful conflict resolution and to build and maintain positive relationships with patients, families, and hospital staff; and develop effective partnerships with community agencies/facilities.
  • Be proficient English language literacy skills sufficient to read and understand all relevant information, investigate available resources, and communicate through written and verbal exchanges with internal and external contacts.
  • Apply HDSHRC values and professional practice standards to patient/family care and service.
  • Perform other duties as required.

The successful candidate will possess:

  • A Bachelor's degree in Social Work or Masters of Social Work
  • Registration in good standing with relevant professional college or regulatory body in Ontario.
  • Three (3) years of recent clinical experience with geriatric or rehab populations preferred.
  • Knowledge of Oracle, Cerner and Millennium will be considered an asset.
  • Proficiency in both official languages (English and French) will be considered an asset.
