Proof of Vaccination Required for Visitors Effective December 22

Effective Wednesday, December 22, 2021 all visitors and essential caregivers coming into Hotel Dieu Shaver will be required to be fully vaccinated against COVID-19, with exceptions only for end of life patients and subject to approval.

As always, our first priority at Hotel Dieu Shaver is the health, comfort and safety of our patients, loved ones, Patient Advisors, staff, and physicians. In light of the pressing circumstances and rapidly increasing Omicron variant surrounding us, this is the natural next step to ensure the highest level of safety for everyone inside our hospital.

Beginning Wednesday, December 22, 2021, all visitors and essential caregivers entering the hospital will be required to show their enhanced vaccine certificate with QR code, along with photo identification. This includes inpatient visitors and essential caregivers, as well as approved caregivers attending outpatient appointments. The enhanced vaccine certificate will be considered the only acceptable proof, as it includes a QR code that will be required to be scanned upon entry. Visitors entering the hospital can show either a printed or digital copy of their enhanced vaccine certificate.
